PAYMENT FORM INSTRUCTIONS

We must receive all payments no later than Friday, August 19.

You can pay by ACH, credit card, or check. There will be a 2.66% credit card processing fee applied to the credit card payments. There is no processing fee for payments by ACH or check.

If you pay by credit card, you must mark the box which says, “Check this box if you are making a credit card payment.”

If you pay by ACH, click the ‘Bank Withdrawal’ radio button and DO NOT mark the box which says, “Check this box if you are making a credit card payment.”

If you pay by check, make the check to Adventures with Mr. Math, write student name(s) in the Memo section, and mail it to 305 Midwest Club Parkway, Oak Brook, IL 60523.

PAYMENT CALCULATION INSTRUCTIONS

If your child is in Level 0-5, click the ADD button next to “Level 0-5 Student.” The Participation Fee for the year includes all costs except the $21 Math Kangaroo contest fee.

If you have multiple children in Level 0-5, click the ADD button next to “Level 0-5 Sibling” for each additional student. You will get a $50 sibling discount.

If your child is in Future Stars or Advanced Review program, choose if you want to enroll your child in one or both semesters and click the ADD button next to the appropriate program.

If your child is a new student, click the ADD button next to “Administrative Fee”. This is the one-time $30 charge per student. If you have multiple children joining the program, click the ‘+’ button to change the count.

If the registration e-mail for your child indicated a credit from last year, you can apply it to your payment by clicking the corresponding ADD button. If you have credit for multiple children, click the ‘+’ button to change the count.
